Where is my 1095-C and What Do I Do with It?

IRS Form 1095C, a form titled Employer-Provided Health Insurance Offer and Coverage, provides information regarding any employer-provided health insurance coverage or offers of coverage.

It details the months of coverage you and any spouse and/or eligible dependents had during each month.

If you were in a benefits eligible position in 2019, you will receive a form 1095-C which will be mailed by March 2. You will also be able to access your 1095-C on the Benefits Platform using the single sign-on link on NLToday.

The IRS does NOT require you to file the 1095-C form with your tax return. You should keep any 1095-C forms you get from National Life for your records.
