IRS Form 1095–C, a form titled Employer-Provided Health Insurance Offer and Coverage, provides information regarding any employer-provided health insurance coverage or offers of coverage.
It also details the months of coverage you and any spouse and/or eligible dependents had during each month.
If you were in a benefits eligible position in 2017, you will receive a form 1095-C which will be mailed by March 1.
The IRS does NOT need you to file the 1095-C form with your tax return. You can keep any 1095-C forms you get from your employer for your records.
