And, yet, too often we neglect to plan for the end of life. Making your personal wishes known significantly increases the chance that you will receive the kind of care that you want in the place you want it.
More important, when time becomes short; your focus can be on doing what you most enjoy and not making hurried, last-minute decisions. The first step is having a conversation.
Jewelene Griffin, RN, Hospice & Palliative Care Manager, and Emily McKenna, Marketing Manager, for Central Vermont Home Health & Hospice, will help us understand the range of end-of-life options and answer questions about appointing a Health Care Agent and filling out the Advance Directive. They will be here to help us fill out the forms.
Our hope is that whoever attends will feel empowered to take the next step. They will distribute conversation guides and relevant paperwork to help us fill out our own advance directives and to feel prepared to talk to family about our wishes.
