Open enrollment season is around the corner. That means biometric screenings, health assessments, and benefits enrollment are all coming up in the following months.
What’s new this year? Vermont’s biometric screenings will take place in the National Life Health and Wellness Center on the second floor (hallway to the right of the lobby elevators). With a new location, we have a new process.
- Notably, if you have not completed the Patient Intake Form for the Health and Wellness Center, you must complete it and return it to the center.
- Next, schedule an appointment with the Center for your biometric screening by stopping in and handing in your Patient Intake Form or by calling 802-229-7515 – or just dial ext 7515.
- Appointments can be scheduled any time now through November 2
- If you want a fasting appointment, be sure to fast for 12 hours prior to the lab work (fasting is not required for the biometric)
- Please allow 20 minutes for your appointment
- The biometrics will be full blood draw, not the finger prick as has been done in the past
Once the labs have been returned to the National Life Health and Wellness Center, you will be contacted to pick up your results. If an appointment is necessary to discuss your results, one will be scheduled at the time you are contacted.
The final piece of the biometric puzzle is the health assessment. You will need to complete your health assessment during the open enrollment process. Your biometric results will the loaded into the health assessment for you.
- The Health Assessment will need to be completed any time between November 6 and December 14 to ensure you receive the premium reduction.
As a reminder, if you prefer to have the biometrics done by your own healthcare provider, you will need to take the Wellness Screening Form with you when you go to your appointment.
